How to switch

A clear transition to a new Plan Manager

Changing Plan Managers should be handled carefully so records, invoices and communication can transition as smoothly as possible. Your existing service agreement may include a notice period, so the first step is to check the agreement you already have.

  1. Review your current Plan Management Service Agreement and notice requirements.
  2. Tell your current Plan Manager that you want to end the agreement in writing.
  3. Contact AUZ Care to discuss onboarding, consent and service agreement requirements.
  4. Authorise the transfer of relevant information where consent or lawful authority is required.
  5. Confirm the transition of current invoices, provider information and Plan Management records.

AUZ Care Service Agreement

Either party may end the AUZ Care agreement by providing fourteen (14) days' written notice. With participant consent or lawful authority, AUZ Care will take reasonable steps to support an orderly transition of relevant information.
